Information Pack
. This pack is aimed at teachers who are including children from Thomas Wolsey school. The pack is aimed to guide you through some of the introductory advice and the people who will be working along side your school with your included child. We hope that it will provide you with some of the basic information about the services we can offer, and explain some of the methods and terms you may come across as you embark on inclusion.   Transition Checklist. The Transition checklist was written by the Outreach Service and the Local Authority to help schools in their planning to admit a pupil with physical disabilities. It covers areas from planning for differentiation of computer equipment to ensuring doors of classrooms are wide enough for all wheelchair users. The document also gives guidance for Suffolk schools on who to ask for help in aspects of Inclusion .Please feel free to use the document in any way that is useful. ICT Assessments
. Below you will find links to five documents.
The first is the Outreach ICT Service Specification and Policy, updated October 2008.
The ICT Assessments leaflet gives details of what an ICT assessment is for and what it involves.
If you then wish to request an ICT assessment, you will need to fill in a Referral Form and an Information for Assessment form and post them to us.
Once you have filled in these forms the Referral Process gives details of what happens next.

IT Training and Support. Training or support is available to schools that have a pupil who is using special software or hardware, usually as a recommendation from an ICT assessment with the Outreach Service.

Intimate care policy. Many children are now being included in Suffolk schools with complex medical and physical needs. Some pupils require Intimate Care within the school day. This document, which was written by the County Child Protection Team, is also attached to the Suffolk SEN policy document as Appendix f. It is available in word format for schools to personalise for their own situation.

Oxygen protocol
. The Protocol was written as a response to requests from Special schools, who had some children within school requiring oxygen. A working party of representatives from the Health Authority, the Advisory service and the Outreach team wrote the protocol to give schools guidelines on safe storage and transportation of oxygen. This document is the Local Authority protocol on the use of oxygen in schools.
